Job Description

Position Summary:  Directs, coordinates, and delivers nursing care to meet the needs of patients who are victims of domestic violence.  Responsible for the provision and on-going development of services to support disease prevention and health promotion initiatives within the medical clinic, working collaboratively with physicians and other team members.  Lead areas of responsibility include care coordination and medical case management, triage and prioritization of walk-in patients; communicable disease prevention; collaborating with the referral team, disease management and prevention.
Work Hours:Rotating shifts – 8:00am-4:00pm & 11:00am-7:00pm; Monday-Friday; Rotating Saturdays; hourly position.

Patient Care:
•Assesses physiological and psychosocial health status of clients, including telephone triage advice;
•Performs patient histories, vitals and screenings for patients.
•Coordinates and administers vaccinations to patients.
•Provides patient health teaching regarding flu prevention, diabetes care, multiple chronic health conditions and promotes healthy lifestyles.
•Oversees patient schedules, appointments, and procedures.
•Manages patient referrals and care navigation; coordinates patient care with physicians and other clinical disciplines in order to address patient needs; monitors patient compliance such as no-show, ER visits, poor labs.
•Performs phlebotomy for laboratory studies and gathers lab specimens.
•Reviews lab and diagnostics and notifies patients.
•Coordinates with the volunteer manager and/or The Family Place’s board volunteer medical doctor as it relates to scheduling the volunteer medical providers and/or medical students.
•Coordinates with outside agencies for on-going care for the patients. 
•Maintains logs for labs, x-rays, sick calls, physician and patient notification and outside referrals.
•Coordinates with outside agencies to provide beneficial community services such as health education seminars, immunization drives, mammograms etc.
Clinic Care:
•Maintains permits, certifications, vaccine temperature logs, patient records, and other administrative functions necessary to operate clinic
•Practices safe storage and transportation of necessary supplies to include vaccines
•Disposes of bio-hazardous waste.
•Determines the need for supplies and orders supplies as appropriate for the health clinic.
•Oversees and supervises the medical closet for clients; supervises the receiving, sorting and storage of OTC meds and donations.
•Prepares and sanitizes examination rooms to include changing paper, sterilizing and setting up instruments and equipment for examining physicians; maintains work area in clean and orderly condition.
•Provides data and/or reports regarding patient diagnosis and demographics in order to collaborate with grant writing and grant proposals.
•Maintains quality and thorough control records.
Other Duties:
•Applies proper procedure for adverse reaction, reporting of incidents and injuries
•Manages policies and procedure manual in conjunction with medical director.
•Completes documentation via the client’s chart and online to an agency approved client database.
•Participates in educational in-services and seminars for health care providers; maintains current certification through continuing education.

Job Requirements

Qualifications: Registered Nurse with active, compact licensure in Texas.  Associate Degree or higher.

Licenses and Certifications:
•RN with active, compact licensure in Texas
•BCLS certification within 6 weeks of hire
•Must have valid and clear Texas Driver’s License

Knowledge, Skills and Abilities:
•Skills should include: taking subjective history, vital signs, CLIA-waved lab procedures such as throat, urine analysis, urine culture, sterile technique and blood draw, wound cleaning, suture extraction, and administrative skills such as correspondence via email and telephone, record keeping, etc.
•Ability to follow strict, standardized guidelines to guard against diseases and other dangers, such as accidental needle sticks, or the chemicals used to sterilize instruments.
•Ability to perform tests according to procedure manual and professional standards.
•Knowledge of current CDC recommendations for immunizations. 
•Ability to draw conclusions from data, such as lab tests, to record this information accurately in patient’s record and collaborate with team.
•Ability to communicate effectively and listen to clients, verbally and non-verbally to establish trust and safety.
•Ability to maintain client confidentiality.
•Ability to multi-task and be a self starter with attention to processes and detail. 

Mental and Physical Abilities:
•Ability to work after hours, when necessary.
•Some duties are performed standing and may be relieved by periodic sitting. Computer or patient file review, etc. duties are usually performed seated.  Sitting may be relieved by brief or occasional periods of standing or walking.
•Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.) frequently.

Working Conditions: Most duties are performed in a well lit, office like setting, with little external noise. May sometimes be exposed to contaminants, infectious diseases, exposed to blood and other bodily fluids. May be required to wear safety attire such as gloves and masks or goggles at times.
